Report and diagnose the effectiveness of these actions on patient commitment & retention to the managed care program (PRM)

Our Team

Our fully integrated team consists of doctors, social workers, nurse practitioners, physician assistants, certified medical assistants, medical assistants, and we will coordinate care, with home healthcare, hospice services, addiction clinics and in patient behavioral health facilities when needed. We always coordinate care as needed in order to provide the best potential outcomes.
